Soro contra veneno de abelhas começa a ser testado em humanos

A primeira fase de testes do soro antiapílico – para combater o veneno das abelhas – chegou à metade do estudo esta semana. O décimo paciente picado pelos insetos recebeu o soro no período de estudo clínico, quando a medicação é testada em seres humanos.

Esta fase do Estudo APIS teve início em fevereiro de 2016 e precisa alcançar 20 pacientes nesta primeira etapa, que tem por objetivo avaliar a segurança do soro. As pesquisas para se chegar a soro começaram há quatro anos, por meio de um consórcio entre o Centro de Estudos de Venenos e Animais Peçonhentos da Universidade Estadual Paulista Júlio de Mesquita Filho (Cevap/Unesp) e o Instituto Vital Brasil.

Em 2013, liderado pela Faculdade de Medicina da Unesp e por sua Unidade de Pesquisa Clínica (Upeclin), o Estudo APIS foi entregue para análise das instâncias regulatórias no Brasil, o sistema CEP-Conep (Comitês de Ética em Pesquisa e Conselho Nacional de Saúde) e a Agência Nacional de Vigilância Sanitária (Anvisa).

Também participam da pesquisa duas importantes instituições de ensino e pesquisa no Brasil: Universidade Federal do Triângulo Mineiro (UFTM – Uberaba/MG) e Universidade do Sul de Santa Catarina (Unisul – Tubarão/SC).

O médico infectologista Alexandre Naime Barbosa, pesquisador clínico do estudo, tem boas expectativas quanto ao soro. “Já aplicamos [o soro] em dez pacientes e não houve nenhum efeito colateral. Ainda não dá para ter grandes conclusões, mas já um indicativo de que ele pode ser promissor. A expectativa é que se consiga fazer um estudo com centenas de pacientes para mostrar a eficácia do soro, que pode ajudar a reduzir o número de mortes”.

A estimativa é que 50 pessoas morrem por ano no Brasil em decorrência de múltiplas picadas de abelhas.

“Quando a pessoa toma mais de 200 picadas, a quantidade de veneno é suficientemente grande para a intoxicação. Cada abelha tem pouco veneno, mas uma concentração muito alta atinge os músculos, gerando um problema sério de sobrecarga renal. Essa é a causa mais comum de óbito em pacientes que sofrem inúmeras picadas de abelhas”, esclareceu Barbosa.

Quando a pessoa é alérgica ao veneno de abelha, uma simples picada pode levar a um grave quadro de alergia generalizada (anafilaxia), e mesmo causar óbito. Nesse caso, as consequências não têm relação com a toxicidade do veneno e o tratamento se limita à medidas para inibir a anafilaxia com drogas antialérgicas.

Em agosto de 2016, o soro foi testado pela primeira vez. A técnica agrícola Camila Aguillar Prezotto tomou mais de 400 picadas de abelhas, enquanto movia um tronco de madeira no sítio do marido em Avaré, no interior paulista. Segundo Camila, o enxame saiu de dentro do tronco.

Após quatro dias de internação em Avaré, ela foi transferida para Botucatu e, em seguida começou a tomar o soro antiapílico. Apesar da dúvida quanto ao soro, Camila decidiu tomar a nova medicação. “Como já estava ruim, pensei na hora em tomar, o que também seria bom para o estudo”. Para ela, o soro foi 100% eficaz. “Desde a aplicação senti diminuir as dores no corpo. As picadas já não doíam tanto”. Segundo a técnica agrícola, o soro não provocou nenhum efeito colateral.

Pesquisa inédita

O desenvolvimento do soro antiapílico é inédito no mundo. “É a primeira vez na história da medicina que se produz soro contra o veneno de abelha. Este é um problema nosso. A abelha africanizada é um tipo das Américas”, informou Alexandre Barbosa.

De acordo com o médico, a abelha africana foi introduzida de forma acidental no Brasil na década de 1950. Ela cruzou com espécies da Europa que já estavam no país e esse cruzamento resultou uma abelha muito agressiva. “Essa agressividade leva ao aumento da periculosidade e do número de acidentes”, afirmou o pesquisador.

A primeira fase de testes termina no próximo ano. “Hoje incluímos nosso décimo paciente. Estamos na metade do estudo. O prazo é final de 2018. Acreditamos que vamos conseguir terminar o estudo para mostrar que o soro é seguro”, disse o médico.

Desenvolvimento do soro

O soro é desenvolvido da mesma forma que os antídotos contra serpentes, com a extração do veneno. “As abelhas passam por uma placa, na qual recebem ondas tênues de choque elétrico. Elas não sentem dor nem sofrem com isso. Ao passar nessa placa, elas despejam um pouco do veneno, que é captado”.

O soro contra veneno de abelhas é heterólogo, ou seja, produzido numa espécie diferente. Após a captação do veneno, ele é injetado na corrente sanguínea de animais, geralmente cavalos, que, com um volume grande de sangue, não se intoxicam com o veneno. O sistema imunológico do animal produz os anticorpos contra aquele antígeno, que formarão a base ativa do soro.
