PESQUISA ABRAF INDICA QUE 76% DOS BRASILEIROS DESCONHECEM A HIPERTENSÃO PULMONAR TROMBOEMBÓLICA CRÔNICA

Dados apontam que 1 em cada 4 entrevistados não sabe como tratar a doença e, em caso de suspeita, menos da metade procuraria um pneumologista

S ÃO PAULO – Sentir falta de ar e fadiga durante a realização de atividades diárias, como subir escadas, pode ser um indício da hipertensão pulmonar tromboembólica crônica (HPTEC), doença incapacitante e que é desconhecida por 76% dos brasileiros . Isso é o que revela pesquisa inédita realizada pela ABRAF (Associação Brasileira de Amigos e Familiares de Portadores de Hipertensão Pulmonar) em parceria com a Bayer, em sete capitais (São Paulo, Rio de Janeiro, Belo Horizonte, Porto Alegre, Curitiba, Salvador e Recife) com 2.100 pessoas.

Pouco conhecida não só no Brasil, mas também no mundo, 5 de maio é o Dia Mundial da Hipertensão Pulmonar.

Para estimular a conscientização da doença, a campanha internacional “Um folêgo para a Vida” apresenta o THUNDERCLAP , plataforma digital que permite que os internautas doem espaço voluntariamente em suas redes sociais (facebook, twiter ou Tumblr) para estimular o conhecimento sobre a doença.

A HPTEC é uma das causas de hipertensão pulmonar (HP), distúrbio que acomete os pulmões e o coração, em que a pressão do sangue nas artérias pulmonares torna-se acima do normal, sobrecarregando o coração, podendo levar à insuficiência cardíaca e até mesmo à morte. Grave e progressiva, ocorre formação de coágulos sanguíneos nas artérias do pulmão que, com o passar do tempo, fibrosam .

Pouco difundida, a patologia ainda é desconhecida para muitas pessoas, inclusive para os próprios portadores da doença, que desconhecem sua existência até que um diagnóstico correto seja feito.

Para a Dra. Gisela Martina Bohns Meyer, Coordenadora do Serviço de Hipertensão Pulmonar da Irmandade Santa Casa de Misericórdia de Porto Alegre, o principal desafio em torno da doença é o diagnóstico precoce, pois muitas vezes os sintomas são confundidos com outros problemas. Assim, muitos casos passam despercebidos e são subdiagnosticados ou recebem diagnóstico tarde, postergando o início do tratamento. Quanto mais tardia a detecção, mais avançada estará a doença e, portanto, mais difícil de responder ao tratamento de forma adequada.

“O paciente pode apresentar falta de ar, cansaço, tontura, dor no peito, desmaios, lábios e pele azulados, pulso acelerado, inchaço nos tornozelos, abdômen ou pernas e dificuldade respiratória mesmo estando em repouso. Algumas vezes, a HP pode ser assintomática e, no momento em que os sintomas aparecem, a doença pode já estar em estágio avançado e não reversível. Na maioria dos casos, entre o primeiro sinal e o diagnóstico correto, o paciente pode levar alguns anos”, reforça a especialista.

Em relação aos GRUPOS DE RISCO , a pesquisa ABRAF questionou os participantes sobre quais perfis eles consideravam mais suscetíveis à doença e, entre as respostas, 39% indicaram os fumantes, 17% optaram por indivíduos com histórico de embolia aguda no pulmão e 8% elegeram os hipertensos. Apesar de mais comum em mulheres e jovens adultos , a HP pode surgir independentemente da idade, gênero, etnia e destes fatores citados anteriormente.

As causas são indefinidas de uma maneira geral, no entanto, alguns casos exigem mais atenção, como: histórico familiar, uso de drogas, doença cardíaca congênita, patologias reumáticas auto-imunes, anemia falciforme, infecções por HIV, doença do tecido conjuntivo e esquistossomose (infecção por parasita). Isso corresponde a uma incidência de 2,4 casos por milhão por ano e uma prevalência de 15 casos por milhão (sendo 15 a 52 por milhão na Europa).

A pesquisa mostrou ainda o desconhecimento acerca das implicações da doença. Cerca de 25% dos entrevistados não sabem dizer quais as possíveis conseqüências da doença . Entre as principais complicações causadas pela doença estão a insuficiência cardíaca e respiratória, arritmias e sangramento dos pulmões.

Para mais da metade dos entrevistados ( 52% ), a vida de uma pessoa com HP é parcialmente limitada não sendo possível realizar atividade física. No entanto, segundo a Organização Mundial de Saúde (OMS), a classificação funcional do paciente com a enfermidade varia numa escala de 1 a 4, que vai desde ausência de limitação da atividade física habitual, passando por limitações discretas a marcantes, até a incapacidade de realizar qualquer tipo de atividade física e a presença de fadiga mesmo em repouso.

O Dr. Caio Julio Cesar dos Santos Fernandes, pneumologista do Grupo de Circulação Pulmonar do INCOR-Hospital das Clínicas da Faculdade de Medicina da USP, ressalta ainda que a doença reduz a capacidade de realizar atividades cotidianas, como tomar banho, vestir-se, andar curtas distâncias e subir escadas. Para a detecção são realizados exames de ecocardiograma, eletrocardiograma, teste ergométrico, cintilografia V/Q scan e outros testes.  “No entanto, para um diagnóstico definitivo de HP é necessária a inserção de um cateter especial sensível à pressão no lado direito do coração (cateterismo cardíaco direito). Recomendamos sempre que, para uma melhor chance de sucesso, pacientes com HP precisam ser tratados em centros especializados”, esclarece o médico.

Tratamento

A pesquisa indicou que 1 em cada 4 entrevistados não faz ideia de como tratar a hptec e, em caso de suspeita, menos da metade ( 47% ) procuraria um pneumologista , já 21% não sabe qual especialista buscar.

O principal tratamento para a HPTEC é a cirurgia. No entanto, 40% dos pacientes é inoperável e, até 35% deles apresenta hipertensão pulmonar persistente ou recorrente após cirurgia. A utilização de medicamentos, apontada por 43% dos entrevistados como a melhor opção de tratamento, é a terapia indicada nesses casos onde o procedimento cirúrgico é contra indicado ou não resolve o problema.

Segundo Paula Menezes, presidente da ABRAF, ” _ o acesso ao tratamento é uma forma de prover dignidade ao paciente. Uma pessoa que não consegue nem pentear os próprios cabelos sem ter falta de ar não se sente minimamente inserida na sociedade”.

Entendendo a hipertensão pulmonar

A doença pode afetar pessoas de todas as idades, incluindo crianças, mas a maioria dos diagnósticos ocorre entre 40 e 50 anos de idade, podendo surgir isoladamente ou com outras doenças subjacentes. Estudos sugerem que a deficiência de óxido nítrico (ON), acompanhada de disfunção do endotélio (o revestimento interior dos vasos sanguíneos), consistem no principal problema da HP e estão associadas à progressão da doença e morte.
